We are looking at improving our delivery of non-invasive ventilation.

We would like to know what head straps/caps are you using and how successful they are?
Also, are you having success with a particular style or brand of mask?

Does anyone have details of an NIPPV mask (available in Oz or elsewhere) that has a port which allows bronchoscopy whilst the patient is recieving NIPPV?

Suggestion, incorporate a suction elbow into the mask and poke bronch through, if leaky you may get some autocycling of nippv machine - we have done this with our CPAP setup - works well .

Not wishing to cause offence but i'm thinking that if a patient's respiratory status is so unstable that they require no interruption to their nippv and on top of that they need a bronchscopy, then surely the airway should be secured by an ett and short term ventilation. under what circumstances to you wish to use such a device?

You'd use it for patients who are managing ok an NIPPV and don't need intubating or patients you don't want to intubate at all (ie CAL with limitations of Rx) where further bronch specimens may help you with
diagnosis ie malig/infection and guide further Rx options. Bronchoscopy may transform this sort of patient who doesn't need an ETT into someone who does if you don't continue their NIPPV
